Policy on the Use of Warner Chilcott's Name, Logos and Trademarks

Warner Chilcott prohibits any unauthorized use of its name, logos and trademarks (together, the "Marks"). Except in connection with materials that are prepared for Warner Chilcott's use at its request, the Marks may not be copied, reproduced, distributed, republished, downloaded, displayed, posted or transmitted in any form or by any means, including, but not limited to, electronic, mechanical, photocopying or otherwise, without prior written permission. Warner Chilcott reserves the right to take appropriate action in the event of a violation of this policy.

Requests for permission to use Warner Chilcott's Marks should be sent to trademark_request@wcrx.com. Please provide the following information with your request:

• your name and company's name;
• contact information including phone number, fax number; email address and mailing address;
• the specific Warner Chilcott Mark that you wish to use;
• background information on your company detailing any relationship with Warner Chilcott, including the names of any Warner Chilcott contacts
• a description of your proposed use of the Mark, including why you want to use the Mark, where the Mark would appear, the duration and the geographical extent of the proposed use, how the Mark would be used in relation to your corporate name and logo and whether the Warner Chilcott trademark would be one of various Marks appearing;
• File type (Adobe, MAC, Vector, etc.) and size required; and
• a description of Warner Chilcott's involvement in any program or project in association with which you propose to use the Mark.

Following submission, Warner Chilcott will evaluate the request and contact you if additional information is necessary. You will be notified in writing within approximately one week whether the request is approved. Warner Chilcott’s failure to respond to any such request shall not be deemed approval. If approval is granted, you will receive written authorization to use the Marks, electronic artwork and usage guidelines where appropriate. Any use of the Marks must be submitted to Warner Chilcott for final approval prior to publication or use. All other use of Warner Chilcott's Marks, except as permitted herein, is expressly prohibited.
